  • Processes and checks all medications, supplies and equipment accurately prior to dispensing from the pharmacy
  • Maintains pharmacy records in accordance with applicable laws and regulations
  • Participates in weekend, holiday and on-call rotation
  • Participates in orientation/training programs for new personnel and current staff
  • Participates in departmental QA activities
  • Attends designated departmental meetings as scheduled
  • Adheres to company policy and procedures
  • Reviews, receives/verifies all new patient orders and documents the same on the respective order form
  • Assists the physician and other healthcare professionals involved in planning and implementing patient care in the facilities to ensure safe and effective drug therapy
  • Conducts initial and ongoing assessment of specific patient, drug and disease information to identify real and potential drug-related problems or needs
  • Assists the physician and nursing staff in setting therapeutic objectives for drug therapy and establishing parameters for monitoring the safety and efficacy of drug therapy
  • Monitors the outcomes from drug therapy by collecting, evaluating, and interpreting the results of laboratory data, clinical data and other pertinent patient information
  • Communicates the results of therapeutic drug monitoring activities to the physician, nursing and other appropriate persons
  • Provides consultation and answers inquiries from patients, families, physicians, and other healthcare professionals regarding drugs and their use
  • Maintains thorough and appropriate documentation in the patient/client record of all clinical activities and communication with patients, physicians and other healthcare professionals
  • Maintains clinical skills and knowledge of pharmacotherapy by reviewing the literature and attending in-services, seminars and conferences
  • Identify and implement appropriate formulary substitutions
  • Matches appropriate drug therapy with diagnosis to ensure efficacy
  • Adheres to company policy and procedures
  • Supervises, coordinates and/or participates in the daily pharmacy work flow and ensures all pharmaceutical and related supplies and equipment are in accordance with federal, state and local laws and standards of practice which govern the profession of pharmacy
  • Supervises training of all new pharmacists, interns and monitors daily work flow and directs personnel as necessary to ensure timely delivery of medications to our customers
  • Reviews staff pharmacists, interns and other personnel as requested
  • Completes a monthly schedule sufficient to meet staffing needs for the pharmacy, while adhering to the operational budget
  • Provides problem solving and decisions related to operations
  • Establish and control back-up pharmacies while adhering to the operational budget
  • Provides leadership and administrative projects and programs
  • Bachelor of Science in Pharmacy (Doctor of Pharmacy or a Masters of Science degree in Pharmacy desirable)
  • Minimum of five years LTC pharmacy experience preferred
  • Demonstrated ability in Microsoft Office-specifically- Word, Excel and Outlook
  • Currently licensed as a registered pharmacist in state of employment
  • Excellent understanding and working knowledge of spreadsheets and reports
  • High level experience with Excel and other database applications
  • Excellent communication, organizational and time-management skills
  • Possess decision making abilities
  • Ability to distinguish medication types (i.e. liquids, creams, ointments, otics, controlled substances, etc.)
  • Knowledge of basic drug terminology, phatmaceutical calculations and drug use
  • Ability to work in a fast-paced environment
  • Ability to work with others in a team environment
