The MDS Traveler (LPN) at CarDon and Associates Corp plays a crucial role in ensuring the delivery of high-quality care to residents in various facilities. This position involves conducting comprehensive assessments and developing individualized care plans that meet the unique needs of each resident. The MDS Traveler will collaborate with interdisciplinary teams to ensure compliance with regulatory standards and improve overall resident outcomes. By traveling to different locations, this role provides flexibility and the opportunity to impact multiple communities positively. Ultimately, the MDS Traveler will contribute to enhancing the quality of life for residents while supporting the mission of CarDon and Associates Corp.
Job Responsibilities
- Conduct MDS assessments and ensure accurate documentation of resident information.
- Develop and implement individualized care plans in collaboration with healthcare teams.
- Monitor and evaluate resident progress, adjusting care plans as necessary.
- Provide training and support to facility staff on MDS processes and best practices.
- Ensure compliance with state and federal regulations related to resident care.
Minimum Qualifications
- Current LPN license in the state of practice.
- Experience in long-term care or skilled nursing facilities.
- Knowledge of MDS assessment processes and regulations.
Preferred Qualifications
- Experience with electronic health record (EHR) systems.
- Certification in MDS coordination or related fields.
- Previous travel nursing experience.
