Regional Registered Dietitian (SNF)

• Provide clinical documentation in the form of nutrition assessments, and high-risk progress notes, and create care plans in a timely and efficient manner
• Collaborate with the Dietary Manager to ensure nourishing and well-balanced diets are provided to meet the needs of each resident
• Participate in Clinically at-Risk meetings weekly
• Communicate with the interdisciplinary team to determine appropriate interventions for the residents
• Assist with survey readiness by completing RD kitchen observations and test trays
• Must be able to travel to different CarDon and Associate communities (Indianapolis south side and north side markets)

Required:

• Baccalaureate degree or other advanced degree in dietetics plus internship
• Must have and maintain current dietitian registration from the Commission on Dietetic Registration (CDR)
• All Registered Dietitians will hold a current Indiana license

Preferred:

• Long term care and nutrition documentation experience
• Ability to effectively collaborate within a team environment
• A passion for working with elderly residents in a caring, compassionate manner
