The RN Clinical Navigator/MDS Staff at Aspen Trace plays a crucial role in ensuring that residents receive comprehensive and coordinated care throughout their stay. This position involves assessing patient needs, developing care plans, and advocating for patients to ensure their rights and preferences are respected. The RN will collaborate with interdisciplinary teams to facilitate smooth transitions and optimize patient outcomes. Additionally, the role includes conducting Minimum Data Set (MDS) assessments to ensure compliance with regulatory standards and to support quality improvement initiatives. Ultimately, the RN Clinical Navigator/MDS Staff is dedicated to enhancing the quality of life for residents through effective care management and patient advocacy.
Job Responsibilities
- Conduct comprehensive assessments of residents to identify their healthcare needs and preferences.
- Develop, implement, and monitor individualized care plans in collaboration with the healthcare team.
- Perform MDS assessments and ensure accurate documentation in compliance with regulatory requirements.
- Advocate for patients and their families, providing education and support throughout the care process.
- Coordinate with various departments to facilitate seamless transitions and continuity of care.
Minimum Qualifications
- Current Registered Nurse (RN) license in the state of practice.
- Experience in long-term care or skilled nursing facilities.
- Certification in Adult CPR.
Preferred Qualifications
- Experience with MDS assessments and care planning.
- Knowledge of regulatory compliance in healthcare settings.
- Strong communication and interpersonal skills.
