About the Role


The MDS Social Service Regional Quality Specialist plays a critical role in ensuring the delivery of high-quality social service care across multiple facilities within the CarDon and Associates Corp network. This position focuses on the comprehensive assessment, planning, and implementation of patient-centered care strategies, emphasizing accuracy in MDS documentation and compliance with regulatory standards. By leveraging data-driven insights and quality improvement methodologies, the role aims to enhance overall resident care experiences and operational efficiency. Ultimately, the specialist serves as a regional resource and advocate for best practices in social service quality, ensuring consistent adherence to organizational and industry standards.


Job Responsibilities



  • Travel to communities to Facilitate MDS assessments and interviews in accordance with RAI manual guidelines and CMS requirements. 

  • Provide ongoing education for social services community teams upon hire and ongoing in social service assessment, MDS, care planning requirements and needs. 

  • Understand and communicate ongoing tracking of community documentation progress with ability to communicate the successes and challenges in the process to community and corporate leadership. 

  • Complete in-depth medical record reviews in specialized areas to identify areas of opportunity in documentation practices. 

  • Reviews care plans and progress notes of a resident’s response to the Social Service interview and complete appropriate assessments, social history and MDS.  


Minimum Qualifications



  • Indiana board certification in social work or a bachelor’s degree in social work or a degree in a human service field (plus one year experience). 

  • Have at least (2) years of experience in a health care setting working directly with individuals. If no degree, then must have a high school diploma or its equivalent and have completed or will complete within 6 months the forty-eight (48) hour social services designee course approved by the State.   

  • Strong leadership and communication skills, ability to foster positive working relationships across multiple facilities and knowledge of MDS and regulatory compliance goals and initiatives is a must. 

  • Basic computer literacy with experience using electronic health records and data management systems.


Preferred Qualifications



  • Experience working within a regional or multi-facility healthcare setting.

  • RAC-CT preferred although MDS proficiency will be considered.
