About the Role


The Memory Care Coordinator at CarDon and Associates Corp plays a pivotal role in delivering specialized care to individuals living with memory impairments such as dementia and Alzheimer’s disease. This position is responsible for developing, implementing, and overseeing personalized care plans that enhance the quality of life and safety of residents within the memory care unit. The coordinator collaborates closely with healthcare professionals, families, and support staff to ensure comprehensive and compassionate care tailored to each resident’s unique needs. Additionally, the role involves monitoring health indicators, managing wound care, and enforcing infection control protocols to maintain a safe and therapeutic environment. Ultimately, the Memory Care Coordinator serves as a key advocate and resource, fostering a supportive community that promotes dignity and respect for all residents.


Job Responsibilities



  • Participates in the inquiry and admission process to screen prospective clients.

  • Welcomes new residents admitted to the special care unit and visits them often to provide psychosocial support and monitor their comfort, concerns, and needs.

  • Participates in care plan development for clients with the Interdisciplinary Team.

  • Carries out or supervises the delivery of care plan approaches on unit assigned.

  • Communicate with other disciplines to evaluate resident responses to interventions, suggest alternative approaches, and determine if needs are being met.

  • Collaborate with Social Services and Activities to deliver these services to residents.

  • Develop working relationships with the local Alzheimer’s Association and participate in events.

  • Facilitates Alzheimer’s support group meetings off-site and/or at the facility.

  • Actively participates in the long-term care survey process. Accompany surveyors and respond to surveyor questions. Provides or offers supplemental information to ensure accurate survey results.

  • Complete any documentation required by nursing policies, regulations, or operational policies.

  • Supervises CNAs providing nursing services to clients assigned.

  • Maintain accurate and up-to-date records using basic computer literacy skills to ensure compliance and facilitate communication.


Minimum Qualifications



  • An active Indiana license as a Licensed Practical Nurse (LPN).

  • A minimum of three years of previous experience is required – two years working with clients who have cognitive impairments.

  • Demonstrated experience in patient care, particularly with elderly or memory-impaired populations.

  • Knowledge of infection control standards and procedures.

  • Basic computer literacy, including experience with electronic health records (EHR) or similar systems.


Preferred Qualifications



  • Experience as a social worker or activities director.

  • Experience working in a memory care or dementia care setting.

  • Additional certifications related to geriatric care or dementia care (e.g., Certified Dementia Practitioner).

  • Training in behavioral management techniques specific to memory care residents.

Qualifications:


* Social Work/Social Services Director with Alzheimer’s care experience preferred!
* Must be knowledgeable about the disease process, changes associated with dementia illnesses & how they affect the individual’s ability to function, and the adaptive strategies that help maintain a resident’s abilities
* Must be able to read, write and follow written and oral instructions in English
* Previous management experience in long term care preferred
